New Patient Registration
North Road West Medical Centre welcomes new patients. Adults registering should normally visit the medical centre personally to sign the registration form – please remember to register all the members of your household.
Alternatively you may print off a registration form, fill it out and bring it in with you on your first visit to the practice.
We will require documentation to verify your identity . The following forms are acceptable - passport, driving licence, birth certificate, marriage certificate, bank card, bank statement, national insurance cared, paid utility (but not a mobile phone) bill
If you are taking regular medication, you should make an appointment for a health check within a month of registering with North Road West Medical Centre.
We cover the city centre and much of the city; our GP surgery catchment area is bordered by the Tamar River to the west and the Plym River to the east. Our northern boundary extends from the Tamar Bridge, along the Parkway, through Budshead Road and Forder Valley Road, re-joining the Parkway with the Waterfront to the south.

If you change your address and it is outside this area, you will need to register with a practice nearer to your new home.
